Living Well with Diabetes

6 Session Group Program | March–May 2026

Living Well with Diabetes is a structured, small group program designed to help you understand your numbers, build confidence around food and movement, and develop sustainable habits that support blood sugar control. This is not a lecture series. It is an interactive, skills based program that blends education, discussion, hands-on learning, cooking demonstrations and real world application.

The program runs over 12 weeks with six in person sessions held Wednesday afternoons in the Crossroads Apothecary Teaching Kitchen. Sessions are led by Keri Connell, CNS, LDN and Roye Evans, RN, CDCES. Participation may be covered by insurance for patient of Crossroads Medical Associates.

    • Introduction to the diabetes meal template and how it applies to daily life

    • Group review of CGM data using a participant’s logs with permission

    • Guided discussion and Q&A around glucose patterns

    • Deeper dive into the meal template, including portioning, timing, and flexibility

  • Exercise and Movement

    • Why movement matters for blood sugar regulation

    • Simple but effective movement strategies that work for real people

    • Live demonstrations with Christina, our in-house health coach, soon to be certified personal trainer and professional bodybuilder

  • Supplements, Medications, and Food as Support

    • Review of common diabetes related medications and supplements

    • How they work, why they are used, and how nutrition fits Supplements, Medications, and Food as Support

      • Recipe demonstration featuring Chia Jam and Greek Yogurt Bark

  • Cooking Class

    • Practical cooking strategies to lower glycemic impact

    • Rice and pasta alternatives including hearts of palm pasta, zucchini noodles, cauliflower rice, and shiratake noodles

    • Focus on taste, satisfaction, and real world substitution

  • Sleep, Stress, and Long Term Success

    • How sleep and stress directly affect blood sugar

Meet your Instructors

Keri Connell, CNS, LDN

Keri Connell is a Licensed Nutritionist and over 15 years of clinical experience. She provides one on one nutritional counseling with a focus on metabolic health, blood sugar regulation, and sustainable lifestyle change. Keri also leads group programs and teaches cooking classes in the Crossroads Apothecary Teaching Kitchen, helping patients translate labs, glucose data, and evidence based nutrition into practical, real world strategies.

Roye Evans, RN, CDCES

Roye Evans is a Registered Nurse and Certified Diabetes Care and Education Specialist with over 20 years of experience in integrative and holistic care. Her background spans nursing, acupuncture, and mind body medicine, allowing her to bridge conventional diabetes education with practical, whole person support. Roye specializes in helping patients understand medications, glucose patterns, stress, and lifestyle factors that influence metabolic health.

  • Getting started is simple. Reach out through our contact form on this page and we’ll walk you through the next steps and answer any questions along the way. Enrollment ends on 3/1/26.

    • Six interactive group sessions over 12 weeks

    • Education on nutrition, movement, glucose data, medications, and lifestyle factors

    • Cooking instruction in a teaching kitchen setting

    • Group discussion and real world application

    • Optional body composition assessments before and after the program using our InBody machine to measure muscle mass and body fat

  • To ensure meaningful outcomes, participants are expected to meet the following requirements:

    • Hemoglobin A1c measured within three months prior to starting the program

    • A second Hemoglobin A1c scheduled within two months after completing the program

    • Attendance at all six sessions, as content builds from week to week

    Optional InBody body composition assessments are available before and after the program but are not required.

  • This program may be covered by insurance if you are a current patient of Crossroads Medical Associates. Space is limited to maintain a high quality group experience. Advance registration is required. Registration ends on 3/1/26.
